Prints coming out bad

I have a CTC 3D printer that the prints are not coming out very well. The top is not covering all the way and the corners are coming up at the top. My bed is set at 65c and the exruder is set at 210c.

The Simplify3D troubleshooting guide is a great place to start. I’m not seeing the images either.
